Covid-19 Information

updated 5/1/21

In order to ensure the safety of both patients and staff we must take certain precautions before and during your visit.  We ask that you follow our guidelines listed below.

When making and confirming your appointment please let staff know if you answer yes to any of the following questions:

  • Have you returned from travel from out of the country or a Covid-19 hotspot within the past two weeks?
  • Have you had any flu-like symptoms in the past two weeks?
  • Have you had a fever, cough, sore throat, breathing difficulty or loss of smell or taste in the past two weeks? Have you been exposed to any one with similar symptoms in the past 2 weeks?
  • Have you been exposed to anyone with a confirmed exposure to Coronavirus?

A facemask must be properly worn at all times while in the building. If you are concerned you may have a fever (low-grade or otherwise) it is best to reschedule your appointment ahead of time to ensure the safety of all of our patients and staff. We ask that you have your paperwork, insurance card, and ID ready when you come in. If you do not have paperwork from our office please let us know in advance.  Please do not bring anyone with you to your appointment unless discussed with staff in advance. Everyone must wear a mask during testing due to the unavoidable close contact throughout the test. You may bring your own, but one will be provided for you if you don’t have one.  For the protection of yourself and others masks must be worn over the nose, mouth, AND chin. If you are unsure when or how to properly wear a mask please review the CDC guidelines below.

Please call us with any questions or concerns.  We appreciate your patience and cooperation while we strive to keep everyone safe and healthy throughout this difficult time.
